Publicado: 08 julio 2022 Historial del artículo....... RESUMEN.- En el presente informe, se describe un enfoque alternativo innovador para la ritidoplastia. La Ritidoplastia a Través de Mínimas Incisiones (Rítmica) es un procedimiento alternativo efectivo, eficiente, seguro y reproducible para el rejuvenecimiento facial. Tiene varias ventajas sobre los procedimientos tradicionales........ OBJETIVO.- El objetivo principal de este informe es describir la técnica quirúrgica de Rítmica...... MÉTODOS.- La técnica quirúrgica para la plastia frontal requiere solo una incisión transversal de ~7 mm ~1 cm detrás de la línea del cabello en la línea media. La blefaroplastia superior e inferior se realizan con incisiones de ~3 mm, y no se necesita sutura. También se puede realizar la liposucción de cuello y el lifting facial mediante incisiones preauriculares...... RESULTADOS.- A continuación, resumimos los resultados obtenidos en 741 pacientes sometidos a ritidoplastia a través de mínimas incisiones. El seguimiento de estos pacientes se extendió hasta 20 años. Esta técnica redujo drásticamente la longitud de las cicatrices; las cejas se elevaron 8-13 mm y, en algunos casos, la punta de la nariz también se elevó ~7 mm. Minimizó o eliminó las líneas verticales del entrecejo y los surcos horizontales en la frente. Por tanto, su venta y publicidad quedaba prohibida en Europa a la espera de la resolución final de la agencia. Ante esta noticia Allergan decidió dejar de vender todos los implantes y expansores con cobertura de Biocell y Microcell. la ANSM (Agence nationale de securité du médicament et des produits de santé) reunirá un comité científico de especialistas para analizar el problema. Posteriormente emitirá su decisión sobre el uso particular de implantes mamarios texturizados. A la espera de sus conclusiones, el conocimiento científico con respecto a las complicaciones de los implantes mamarios se pueden resumir de la siguiente manera: 1. Complicaciones poco frecuentes como la contractura capsular. 2. La relación causa-efecto de los implantes con el linfoma anaplásico de célula grande (LACG-AIM o BIA-ALCL). 3. La dudosa relación de los implantes mamarios con el desarrollo de enfermedades autoinmunes. La Asociación Mexicana de Cirugía Plástica realizó una encuesta para determinar los factores relacionados con la muerte súbita en lipoaspiración. En el periodo de 2005-2014 se reportaron 64 casos; con un incremento de 1,479 casos por cada año (R = 0.983; p < 0.001), se pronosticaron 18 muertes para el año 2015. El mayor porcentaje de mortalidad relacionado con los años de experiencia de los anestesiólogos fue entre 15 a 20. Hubo 12 fallecimientos antes de iniciar el procedimiento quirúrgico. El mayor porcentaje de mortalidad en cuanto a los años de experiencia de los cirujanos fue de los 20 a los 25. Los procedimientos fueron realizados 15.62% en consultorio, 54.68% en clínicas y 29.68% en hospitales. Las manifestaciones clínicas fueron en el 68.73% hipotensión arterial, disnea con tos, bradicardia y desaturación de oxígeno. Las principales causas de muerte fueron embolismo graso 26.56%, tromboembolia pulmonar 20.31% y desconocida 31.25%. La lidocaína fue utilizada en la solución tumescente en 59.37% de los casos a dosis de 13.88 a 80.64 mg/kg peso. Hubo una fuerte correlación entre la aplicación de lidocaína y el tiempo de muerte transoperatoria; la mayor mortalidad fue en los primeros 60 minutos (B = -2.63% R = 0.924; p = 0.008) y en el postoperatorio la mayor mortalidad se presentó a las 4 horas y se abatió a las 16 horas (B = -4.4% R = -0.917; p = 0.005). Hubo un total de 38 casos (59.37%) de fallecimientos que pudieron haberse prevenido. Se establecen recomendaciones preventivas. Rhytidoplasty Through Minimal Incisions (Rhythmic) is an effective, efficient, safe, and reproducible alternative procedure for facial rejuvenation. It has several advantages over traditional procedures.... OBJETIVE .- The main objective of this report is to describe the surgical technique of Rhythmic.... METHODS.- The surgical technique for the foreheadplasty requires only a transverse ~7-mm incision ~1 cm behind the hairline at the midline. The upper and lower blepharoplasty are made with incisions of ~3 mm, and no suture is needed. Liposuction of the neck can also be performed, and the face-lift can be achieved through preauricular incisions..... RESULTS.- Here, we summarize the results obtained in 741 patients subjected to rhytidoplasty through minimal incisions. The follow-up of these patients extended up to 20 years. This technique drastically reduced the length of scars; the eyebrows elevated 8-13 mm, and in some cases, the tip of the nose was also elevated ~7 mm. Whit the limitations of surgical reconstructive procedures, the growing number of gunshot wounds, burns, and work accidents in Mexico that result in complex facial deformities leaves only 1 option – face trasplantation. The National Institute of Medical Sciences and Nutrition “Salvador Zubiran” ( INCMNSZ) has performed transplants since 1971. We at INCMNSZ undertook the 1st bilateral upper-limb transplantation in Latin America in 2012. We are willing to continue in this manner toward conducting face transplantation at aour institute. To this end, we identified and solved various challenges. The Ist challenge was acceptance and inclusión of vascularized composite allotransplantation (VCA) within general Mexican health law and approval of the face transplantations procedure. Subsquently, the health ministry provied a license to INCMNSZ to perform the procedure. The 2nd challenge concerned who would pay for procedure. The costs Will be paid by the patient (1st-partypayer), social security institutions (2nd-party payers), and the health ministry (3nd-party payer. The 3rd challenge was to maintain ongoing sirgical training of the team using cadavers. The fourth challenge was to locate donors: toward this end, we developed a compaign for promoting face donation in social media, making a comic book, and training organ and tissue coordinators to further VCA . Thus, INCMSZ has the legal, administrative, medical, and surgical wherewithal to accomplish face transplantation. However there appears to be an increase from 0.86 to 1.06 per 100,000 populations in the last 26 years...///// ...... SUDDEN DEATH IN PATIENTS OF LIPOSUCTION *** The Mexican Association of Plastic Surgery conducted a survey to determine the factors related to sudden death in liposuction. There was a strong correlation between the application of lidocaine and time of death during surgery; the highest mortality was in the first 60 minutes (B = -2.63% R = 0.924, p = 0.008) and postoperative mortality was greater at 4 hours and lower at 16 hours (B = -4.4% R = -0917, p = 0.005). There were a total of 38 cases (59.37%) of deaths that could have been prevented. Preventive recommendations are established. Current studies are not conclusive in fInding causes.....////// .... LIP AND PALATE HAS A GREAT IMPACT ON PUBLIC HEALT ***The prevalence of cleft lip and palate has a great impact on public health, as surgical correction may require from one to five or more interventions. The objective of this study was to determine the frequency of cleft lip and palate cases in Mexico reported by the General Directorate of Epidemiology of the Ministry of Health through a channel to position endemic records during the months of 2015. An observational, descriptive, transversal and retrospective study of lip cases detected in Mexico, reported by the General Directorate of Epidemiology of the Ministry of Health of Mexico from 2008 to 2015 was carried out. In Mexico, 799 new cases in males (60%) were recorded, while 529 female cases corresponded to 40%. The months with more reported events were March and July where the expected events were positioned in the warning area. We concluded that the majority is in the masculine gender, and March and July are the months with more recorded events
